Change sign-in form to choose-name form

Sign-in form should become choose-name, it includes:

• Change the name of the form
• Remove all text fields but email field
• Rename email field to "name" field
• Remove email validation in name field
• Change the text of the sign-in button
• Remove sign-up button in the bottom of the form
